Book Appointment

No referral is required to access our diabetes and cardiac education services. You can contact us directly to book an appointment.

A referral may be helpful if:

• Your doctor wants to provide additional health information
• You're accessing specialised services (insulin titration, MDT program)
• You'd like your GP to be informed about your progress

Call our team in either Darwin or Alice Springs

If you have a referral:

We may have already received it. Our educators review all referrals and will make a recommendation to our front desk staff for your appointment with us. 

If we have not received your referral, please sent it to us or bring it along to your appointment. 

If you do not have a referral: 

You do not need a referral to access our services. Speak with our reception team and/or educators and we will help find the best option to support your health needs.
 

Need an interpreter? Please let our reception staff know when booking so we can arrange one for your appointment.

Make note of any questions you have. Family members, partners and carers are welcome and encouraged to attend.

What to bring:

• Any recent pathology results, if you have them
• List of your medications
• Any other medical information
• Medicare card

Please make sure to arrive on time to take full advantage of your allotted appointment time. 
We recommend arriving 5 minutes early to complete any paperwork required.
